422 unprocessable error

We followed the installation instructions and have a QPixel standalone instance with seems to be working. However we are not able to create the first account. The sign-up process results in a "422 unprocessable" error.

A comment in post below mentions: "This is also what Rails uses when a resource couldn't be processed because of validation errors."
